Try one of these Handcrafted Thumb Book Holders and simplify the process. Use with the pointy side facing into the fold.  Made using Irish Spalted Beech that originated in Co Galway Ireland.

Size approximately  80mm x 34mm inches at the widest points.
 
The Thumb Hole is 24mm wide and I recommend lining the back of your nail with the middle of this device.  Your thumbnail should only be poking out a little ways through the front. Never push your thumb through if it is a tight fit, instead only use a  fraction of your thumb. The size of the hole is meant to be almost universal. If in doubt, please ask a question.
